This form is for use by veterinarian clinics and pet insurance providers only.

You can use this form to formally request a patient’s medical history and notes. You will be required to declare that you have consent from the pets owner to whom the medical notes pertain. Please fill in one form per pet. Once you have submitted the request, your application may take up to 24 – 36 hours for internal procedures to be carried out and the medical notes to be released. Medical notes will not be forwarded to private addresses.
